Job Description

Adecco Recruitment in Bolton are currently recruiting for on behalf of our Client a Social Services Care Coordinator to work for a successful care agency in the Bolton area. The ideal candidate will have worked as a carer before and also have a strong administration background as this role involves working in a busy office and coordinating care staff to work within assigned geographical area. In some cases this role might involve care work, so this is why we are looking for previous care experience.

JOB DESCRIPTION CO-ORDINATOR

MAIN PURPOSE: To enable Service Users to remain in their own homes by providing the highest standard of support and care required to help them achieve maximum independence whilst maintaining their self-respect and dignity

RESPONSIBLE TO: Registered Manager


Main Duties:-

1. Adhere and implement all Company policies and procedures, including Equality and Diversity, to ensure the aims and objectives of the organisation are met

2. To input and keep up-to-date customer and staff information on computer software system

3. To assist the branch manager in all aspects of care management in the designated area

4. To ensure that wages information is correct in order that the invoicing department produces accurate invoices

5. Assist with problems that may arise between Care Workers and Service Users effectively on a daily basis

6. To act at all times in a professional manner as a representative of the Company and to support its good reputation

7. To co-ordinate Care Staff and resources & monitor individual care packages whilst matching Care Staff to Service Users to provide a person-centred service

8. Ensure the efficient and effective delivery of services to all Service Users

9. Liaising with Social Services and all effective social care agencies, working efficiently to provide effective support to clients.

10. To compile and maintain accurate records and ensure they remain up-to-date on at least a daily basis

11. To ensure the service delivered meets the requirements of the Service User

12. To ensure that all requirements of Health and Safety legislation are met and the Registered Manager is informed of any incidents that have arisen

13. To raise awareness of the complaints procedure and where action is necessary, report immediately to the Registered Manager.

14. To participate in the out of hours, weekend and Bank holiday 'on-call rota' when necessary

15. Ensure all associated administration is in place through liaison with the office staff

16. Maintain regular contact with Service Users by telephone to regularly monitor and review the effectiveness of the care package

17. To respect Service User and staff confidentiality in accordance with Company policy & the Data Protection Act

18. To be accountable for your work practice and take responsibility for maintaining and improving your knowledge and skills by attending ongoing training as requested by the company

19. To follow the complaints procedure in cases of dispute and to report all accidents/incidents, no matter how minor, to the Registered Manager

20. Ensuring that holiday/sickness and emergency calls are assigned

21. All other appropriate duties that fall within the purview of the post as may be agreed
